Mechanism of Urine Formation

Urine formation is a crucial physiological process carried out by the kidneys to remove metabolic waste, excess salts, and excess water from the body. This process helps maintain the internal balance of fluids, electrolytes, and pH in the body, thereby ensuring proper functioning of cells and organs. The functional unit of the kidney responsible for urine formation is the nephron. Each kidney contains millions of nephrons that work together to filter blood and produce urine.

Urine-Formation

Urine formation occurs in three important steps: Glomerular filtration, Tubular reabsorption, and Tubular secretion.

1. Glomerular Filtration

Glomerular filtration is the first step in urine formation. It occurs in the glomerulus and Bowman’s capsule. The process of Glomerular Filtration occurs as follows:

  • Blood enters the glomerulus through the afferent arteriole. The diameter of the afferent arteriole is wider than that of the efferent arteriole. Due to this difference in diameter, high hydrostatic pressure is created inside the glomerulus. This pressure forces water and dissolved substances out of the blood through the thin walls of the capillaries.
  • The substances filtered into Bowman’s capsule include Water, Glucose, Amino acids, Salts, Urea, Uric acid, Ions, and Small molecules. The fluid that enters Bowman’s capsule is called glomerular filtrate.
  • The filtration membrane consists of the capillary endothelium, the basement membrane, and the inner lining of Bowman’s capsule. This membrane acts as a selective filter.
  • Large molecules such as Blood cells, Platelets, and Plasma proteins cannot pass through the filtration membrane and therefore remain in the blood.
  • The amount of filtrate formed by both kidneys per minute is called the glomerular filtration rate. In a healthy adult, the GFR is about 125 mL per minute. Approximately 180 litres of filtrate are formed every day, but only about 1 to 1.5 litres of urine are finally excreted because most substances are reabsorbed.

2. Tubular Reabsorption

Tubular reabsorption is the second step in urine formation. During this process, useful substances from the filtrate are reabsorbed into the blood. This process mainly occurs in the proximal convoluted tubule, although reabsorption also occurs in other parts of the nephron. Reabsorption in different parts of the Nephron:

Proximal Convoluted Tubule (PCT)

The proximal convoluted tubule is highly coiled and surrounded by blood capillaries. It reabsorbs most of the useful substances. Substances reabsorbed in the PCT include Glucose, Amino acids, Vitamins, Sodium ions, Chloride ions, Bicarbonate ions, and Water. About 70% of water and salts are reabsorbed in the PCT.

Loop of Henle

This arrangement creates a concentration gradient in the medulla, which helps in concentrating urine. The loop of Henle has two limbs:

  • Descending Limb: The descending limb is permeable to water but not to salts. Water moves out into the surrounding tissues by osmosis.
  • Ascending Limb: The ascending limb is impermeable to water but allows salts to move out.

Distal Convoluted Tubule (DCT)

  • It selectively reabsorbs sodium ions under the influence of aldosterone, helping regulate blood volume and blood pressure.
  • It reabsorbs water under the influence of antidiuretic hormone (ADH), thereby contributing to the concentration of urine.
  • The distal convoluted tubule reabsorbs Sodium ions, Calcium ions and Water. This process is regulated by hormones.

Collecting Duct

  • It receives filtrate from the distal convoluted tubules of several nephrons and carries it toward the renal pelvis.
  • It reabsorbs water under the influence of antidiuretic hormone (ADH), thereby concentrating the urine and conserving body water.
  • It reabsorbs sodium ions and secretes potassium ions under the influence of aldosterone, helping maintain electrolyte balance and blood pressure.
  • The collecting duct reabsorbs water according to the body’s needs.

3. Tubular Secretion

  • Tubular secretion is the third step of urine formation. In this process, certain substances are actively transported from the blood into the renal tubule.
  • Tubular secretion mainly occurs in the distal convoluted tubule and the collecting duct
  • Substances Secreted are Hydrogen ions, Potassium ions, Ammonia, Creatinine, Drugs and toxins, and Excess salts
  1. Kidney Stones: Kidney stones are hard deposits formed due to the accumulation of salts and minerals in the kidneys.
  2. Uremia: Uremia is the accumulation of urea in the blood due to kidney failure.
  3. Diabetes Insipidus: This disorder occurs due to insufficient secretion of ADH, leading to excessive urine production.
  4. Nephritis: Inflammation of the nephrons is called nephritis. It affects the filtration process.
